- This plan is only honored at Nightingale Family Dental. It cannot be used at any other dental or specialist office.
- This plan cannot be used for hospitalization or hospital charges of any kind.

- Any unused services within the patient membership are nontransferable to other plan members regardless of the service.
- Any missed appointments or cancellations not made within 48 hours of the appointment may be charged a cancellation fee.
